Welcome to the OG Elite Basketball season. To ensure a safe, disciplined, and elite environment for our athletes, we require all families to review and complete the following documentation. Action: All players and parents must read and sign this agreement to participate.

Our program is built on the "Patriot Pipeline"—a commitment to hard work, integrity, and teamwork. Please ensure all forms are submitted prior to the first practice.

1. Values and Code of Conduct

At OG Elite, we hold our coaches, players, and parents to a professional standard. This document outlines our expectations for sportsmanship and accountability both on and off the court.

2. Medical Release & Liability Waiver

The safety of our athletes is our top priority. This form provides our coaching staff with essential emergency contact information and the medical authorization required to ensure your child is cared for in the event of an injury.

3. Team Parent & Game Day Guidelines

We believe that "many hands make light work." This document explains how parents can get involved, from managing the game clock to assisting with our high-tech game film and GameChanger stat tracking.
